ASSIST - PERSONAL ACTIVITIES

Support Coordination
At Nova Horizon Sport Services, we offer professional support coordination services to simplify access to the National Disability Insurance Scheme (NDIS) for individuals.

Our experienced team have a medical background and is highly trained in delivering care for the most complex of cases, including care for people with physical, mental health and behavioural disabilities.
Assist Personal Activities High
If you’re being discharged from hospital you may need extra help. Our Hospital to Home Discharge Service can provide all the support you need during your recovery

The aim is to enhance their overall well-being, independence, and quality of life by fostering meaningful connections with others and being actively involved in community life
Development Of Life Skills
We enhance existing skills by motivating participants to design routines that allow their particular skillset to be used regularly.
